Job Description Job Description Description: I. POSITION TITLE
Direct Support Contractor – Personal Supports Division (Hereafter
referred to as “DSC” or “Contractor”) This title is used because
it: · Aligns with national Direct Support Professional standards ·
Meets APD’s definition of a Direct Service Provider under Chapter
393 · Applies across all FTC service lines under the Medicaid
Waiver & FCC Pilot · Maintains contractor legality and independence
· Supports a statewide/national audit-ready standard II. POSITION
PURPOSE The Direct Support Contractor provides skill development,
personal assistance, supervision, and community access services to
individuals with developmental disabilities participating in
Medicaid Waiver and FCC Pilot Program services under FTC. This role
exists to ensure health, safety, independence, and rights
protection through compliant service delivery in accordance with: ·
Chapter 393, F.S. · Florida Administrative Code Chs. o 65G-2
(Licensing) o 65G-4 (Behavioral Services) o 65G-7 (Medication) o
65G-8 (Reactive Strategies) o 65G-10 (Training) · iBudget Waiver
Handbook · APD Operating Procedure 3-0006 (Incident Reporting) ·
FCC Ancillary Agreement · Qlarant Administrative Service-Specific
Review Tools The DSC operates as a professional, independent
contractor , not an employee of FTC. III. SCOPE OF SERVICES This
position provides services across FTC program areas including: ·
Personal Supports · Life Skills Development I, II, III ·
Residential Habilitation (Standard, BF, IB) · Adult Day Training
(ADT) · Transportation (non-medical) · Behavioral Services Support
(non-clinical prompting only) The contractor’s activities must
always remain within the boundaries of APD and FCC scope of
practice and must never substitute clinical, medical, or restricted
behavioral functions. IV. ESSENTIAL DUTIES & RESPONSIBILITIESA.
Direct Support Functions The Contractor must: 1. Provide
individualized support with ADLs and IADLs , including: o Eating,
dressing, bathing, grooming, toileting o Meal preparation,
housekeeping, laundry o Community mobility, safety, and social
integration 2. Support the individual’s goals and outcomes as
listed in: o Support Plan (SP) o Implementation Plan (IP) o Risk
Plans o ADT skill goals (if applicable) 3. Assist in community
integration , including: o Public transportation training o
Community safety o Shopping, budgeting, banking assistance (without
handling funds) 4. Encourage self-determination , personal choice,
autonomy, and independence. 5. Follow all behavioral
recommendations that fall within the PSC scope, including: o
Redirection o Visual prompting o Environmental modification NOTE:
All reactive strategies must comply with 65G-8 . No manual
restraint, mechanical restraint, or chemical restraint may be used.
B. Documentation & Medicaid/FCC Compliance The Contractor must
complete Qlarant-compliant service logs including: · Person served
· Service type · Date · Time in/out · Location · Detailed narrative
of service delivery · Signature The Contractor must: · Submit
documentation according to FTC’s Independent Contractor Invoice
Policy · Maintain documentation for six years · Follow all Medicaid
Waiver and FCC recordkeeping requirements · Provide documentation
necessary for Qlarant, APD, FCC, MCO, or FTC audits · Maintain
copies of all plans and updates relevant to service delivery C.
Health, Safety, and Rights Protection The Contractor must: ·
Protect and uphold the Rights of Persons with Disabilities (393.13,
F.S.) · Ensure a safe environment and report hazards immediately ·
Follow all risk mitigation and emergency procedures · Ensure
supervision consistent with the Support Plan and assessed needs ·
Immediately report: o Abuse, neglect, exploitation o Critical or
reportable incidents o Missing persons o Behavioral crises o
Medical emergencies Reports must follow APD OPS 3-0006 , FTC’s
Incident Reporting Policy, and FCC requirements. D. Professional
Standards The Contractor must: · Maintain professional boundaries
and confidentiality · Uphold HIPAA and all privacy obligations ·
Communicate professionally with FTC supervisors, WSCs, FCC
coordinators, and guardians · Participate in required training
mandated by APD, FCC, CMS, and FTC · Submit to monitoring,
auditing, and compliance reviews Requirements: V. MINIMUM
QUALIFICATIONS 1. Minimum 18 years of age 2. Completion of all
APD-required Basic Training and service-specific training 3. Level
II Background Screening through the APD/DCF Clearinghouse 4. Valid
driver’s license (if providing transportation) 5. Ability to read,
write, and document services professionally 6. Ability to follow
structured programs, Implementation Plans, and supervision 7.
Physical ability to perform ADL support tasks safely VI. CONDITIONS
OF ENGAGEMENT (Independent Contractor Status) The Contractor: · Is
not an employee of The Family Tree Concept, Inc. · Must provide a
valid W-9 · Is responsible for taxes, insurance, and professional
tools/equipment · Must comply with the FTC Independent Contractor
Agreement & Schedule A Failure to maintain compliance may result in
immediate termination of the contract. VII. PERFORMANCE STANDARDS
FTC evaluates the Contractor using: · Qlarant Administrative &
Service-Specific Standards · Documentation accuracy and timeliness
· Compliance with APD, FCC, and CMS requirements · Adherence to
Support Plan goals · Incident reporting compliance ·
Professionalism and reliability · Safety and rights protection
VIII. REGULATORY REFERENCES This position is governed by: · Chapter
